Believe it or not, getting AK47'd in the chest really didn't hurt. Like a bee sting or a cigarette burn, which is why I thought it had just brushed me a little. I took a 9MM ricochet in the meat of my calf another time and it hurt like hell right away. Real injury is weird like that – what is most damaging may not hurt as much as something minor.
Anyway, other than that anecdote I don't have much to add to your excellent recommendations, except that mental failure always precedes physical failure. You *can* do one more rep or one more mile, your nervous system just doesn't want you to. Whatever limits you set for yourself are real because you believe they are and you can choose to ignore them – just ask Roger Bannister of Fred Hatfield. The 4 minute mile and 1,000 pound squat were overcome because they elected to ignore the fact that they were 'impossible'.
