The other doc in my office has seen a case just like this (test-wise), and I'm dealing with two people right now with test levels in the 400s (again, late 20's).Questions for you about the Crossfit world.
I have a few guys who have come in for hormone testing, big Crossfit guys, and they are trying to tell me that Crossfit will lower your testosterone. They couldn't really elaborate on this much and believe their low testosterone is ok, so I figured I'd ask you.
I assume they are saying the workouts are so hard that they burn up their testosterone during the workout and if you test them anytime during the day, they will be low. This isn't how I remember hormones working, yet I figured I'd ask you, the "workout guy".
Most of them are 25-28, their testosterone levels are just over 400 total, 10% free, SHBG is at least 30 (I've seen up to 50). Perhaps Crossfit is bad for your hormones.

One of the worst things that CF promoted in training is the idea that every day needs to be PR day. Going max effort on every single workout can't be good for the body over time.

Many more studies out there with similiar results.
Not a simple one variable answer, but intense (meaning very high level of effort workout sessions) impacts test levels greatly, as do endurance workouts.
