Culture is a thing. Cultural diversity means low trust and social friction. Look at Tokyo vs NYC. Look at Sweden pre and post arab immigration.
Canada, for one, is a single-payer state with a more diverse population than the US, ethnically and culturally.
Spain - while all roughly the same skin tone, I think the Basques and Catalans would like to discuss shared culture with you

The Flemish and Walloons? Possibly more divided than American 'coastal elites' and 'heartland patriots' culturally. Italy? Barely even a nation-state. Hell, a country as tiny as Switzerland has culturally French, German and Italian regions. Switzerland is the size of a couple of our smaller states combined.
Note, all of those states pay less for healthcare for superior outcomes.

All of them also have reasonably competent health care bureaucracies. We don't and, as the Obamacare experience has proven, our healthcare bureaucracies are remarkably resistant to change.
