Re: bend over penn state
The Jim Jordan bit is controversial, the doctor being a complete creep is not. But he's dead, fortunately for him.

Re: Bright on Netflix
Loved it. I think it punched way above it's weight. David Ayer makes entertaining movies. It reminded me of Shadowrun, if anyone remembers that, from my secret nerd past. https://en.wikipedia.org/wiki/Shadowrun Man, I loved the Shadowrun concept. The gaming system, meh, but this will pretty much se...

Glad you are OK. I may have missed it in the piece - did you get an echocardiogram of your heart?

Re: Anyone tear a hip labrum?
I was hoping for a torn labrum, but turns out I had worn down to bone on bone arthritis, ended up with hip resurfacing (instead of replacement). Agree with prep glute work, perhaps deadlifting off of pins to keep the posterior chain going without the stress on the hip capsule from dropping deep into...

Flu shot. Every year, would even if it weren't required and free. I put people on life support every frickin' year that turn out to have the flu. The "I get the flu when I get the flu shot" argument. No, you don't. The flu shot engenders an immune reaction, a distant early warning for your...

Don't move to Asheville, where apparently low income hipsters go to cry about not making good money This is truth. The excellent quality of the local breweries, while still quite good, has now been met or exceeded in less intentionally ironic portions of the state. Although Asheville remains a grea...

Re: Med professionals: ? On ethics
adderall and xanax...fun stuff. No med board in the country will see that as legitimate. First time offenders will usually get a reprimand, possibly a fine, and may have to take a CME course on ethics. Unless, of course, there is evidence that the prescription for Uncle Bobby is actually being dive...

Re: Med professionals: ? On ethics
Most medical boards allow for "time limited illness" - prescribing a course of antibiotics, or anti-puke meds. But long term meds should be handled by an objective 3rd party. Controlled substances - no frickin' way.

Johno, Community hospital will fall apart with one. Prepared academic center - we've had three false alarms - one transfer, one from a community physician's office, one primary EMS call. I will not detail them except to say that each one has provided insight and improvement on a plan that was declar...

Re: ebola quarantine--governmental overreach?
The guidance on personal protective equipment has continued to evolve from facemask and gloves up to full gear / no exposed skin. I go in for my refresher course tomorrow. It is very easy to self-contaminate while removing a standard facemask and goggles (demonstrated by UV light testing by EMS here...

I have had the precordial thump work, but the normal heart rhythm only lasted about a minute as I did not hit him hard enough to dislodge the clot in his coronaries. Re: wilderness CPR. If it is a heart issue (as opposed to a breathing issue due to Shape misdosing his "evening cocktail") -...

Re: Do you know CPR?
Buttercup is correct. Uninterrupted compressions increase the blood pressure and delivery to the coronary arteries, which feed the heart muscle. Stop the compressions (to give a breath, to check for a pulse) and the perfusion pressure plummets, starting from zero when the compressions restart. Plus ...

Re: Gabapentin and sciatica?
The evidence for gabapentin for nerve based pain is less than robust, and any time you are mucking about with neurotransmitters you run the risk of weird and unpleasant side effects. My thoughts would be piriformis stretching as above, gabapentin if absolutely needed until the stretches kick in, NSA...

Inguinal hernias are those that are in the groin. Umbilical hernias - behind the belly button. There are other varieties that are rarer. Or, it could just be a connective tissue strain.

Re: Let's Remember 9/11
That morning was our weekly educational conference for the emergency medicine residents at Ohio State. Our medical director came in and announced that the towers had been attacked, and that we were all to stand by on campus to prepare to receive casualties. The hospital turned on CNN on one of the a...
