PS - My strong speculation: Many of the fifty+ deaths occurred because it took three hours for the police to end the incident. The Golden Hour applies to gunshot wounds.

I instruct my students each year that if someone comes in shooting, the path to life is to throw everything you have at his eyeballs while rushing him, and then when you get to him pile on until he is motionless, and that while some of you will get shot and die, hopefully most will live. The 3 hour wait is unfathomable. I though we'd learned from Columbine to go in fast. They surely could have saved some of those victims.

Actually, lots of states do allow you to have a gun in a bar. Virginia does as of a few years ago, you just can't drink if CCing. I don't recall it ever causing any trouble.

There's one SOP for a hostage situation, another for Active Shooter. Apparently, Orlando SWAT treated it like a hostage situation. Maybe it was at that point.
Until it wasn't and they breached the building.
BUT, my impression is that the wounded in the building didn't get medical attention when the shooter was barricaded in the bathroom with hostages, which would call for a sort of hybrid response: secure part of the nightclub to treat victims and contain the shooter in the bathroom. (I know this can be done - I've done something similar.)

Shockingly i agree with Spells that guns can't be in a bar. As much as i am pro gun and pro carry that seems a bit too far. Firewater and firearms a bad mix. But on the other hand i rarely go anywhere i can't carry or doesn't have metal detectors. And don't stay anywhere that we can't GTFO of quickly if required.

But just being in a normal establishment that has alcohol while armed has not turned out to be a big deal. Every state that has changed the law to allow concealed carriers to be inside bars or restaurants, the prediction has been that the floors will run red with blood. And it's been wrong every time. It turns out licensed carriers try real hard to stay out of trouble. The only times we had violence at clubs here it was always illegally armed prohibited persons. The local establishments decided to try and reduce the chance of shootings/stabbings by no longer hosting Hip-Hop events. This tactic has proved very successful. Better than metal detectors and off-duty police. Caused a lot of local hand-wringing about prejudice,
